Please fill out this form completely and print two copies. Send one copy with your your EBC and keep one for your records.

Once your EBC is received it will go into our system. Depending upon the condition of your EBC it will be updated and shipped out within 2-3 days. Our goal is to get them updated, filled, and shipped out the day after they arrive. If your boost controller is not operating correctly or has water damage it may slow down the process as it takes some time to dry, clean, and repair the EBC. In the event it can not be repaired we will ship out a new boost controller.

Only send the Electronic Boost Control Head Unit, we do not need the harness or bracket or any other parts that came with your EBC.
All fields are required.
Before submitting your form be sure all fields are complete or it will refresh the screen and you will have to refill out the form. If form is completed correctly you will be see a "thank you for submitting your form" message on the screen.
Please print 2 copies of this form, one for your records, and a copy to send in with your EBC.
